Filling out Your DE 964 Completion Certificate
For Parent Taught Driver Education Students
If your completion certificate doesn't look similar to this one or request the same information, you need to contact your course provider and ask for the "completion DE 964". Most course providers will email this to you.

- Student name should be exactly as it appears on your birth certificate, legal adoption documents, court order or passport.
- All dates on this form should be chronologically before the date on your Impact Texas Teen Driver Certificate.
- The parent who was designated as your instructor when you filled out your TDLR paperwork is the one who signs this form. There are two places for this parent to sign the document and include their driver license number.
- If you transferred from a driving school to parent taught, you will need to also bring the transfer document
Transfer Document
If you started at a driving school for your classroom instruction, this form comes from the previous driving school to show you completed the classroom portion. If you did not get one from your previous driving school, contact them and let them know you need the "transfer DE 964".
